Guidelines on Online Enrolment and Payment of Tuition and Other Fees

New Students

  • Step 1: Application for Enrolment

    • Visit www.mham.edu.ph and select Online Enrolment and download  application forms (New | Transferees).
    • Submit all admission requirements, both original and duplicate copies at the Dean’s Office.

    • Pay the enrolment application fee of P1,000.00 to any of our designated banks found in the third page through over–the–counter transaction under Account Name:  MATIAS H. AZNAR MEMORIAL COLLEGE OF MEDICINE.

          Admission Requirements for Incoming 1st Year Students:

    • Birth Certificate: 1 original, 1 photocopy
    • Diploma from a Bachelor of Science Degree Course:  1 authenticated, 1 photocopy.
    • Authenticated Transcript of Records: 1 original, 1 photocopy. (Transfer Credential)
    • Certificate of Good Moral Character: 1 from the Dean, 1 from the Professor of previous
      school attended.

    • NMAT Certificate of not more than two (2) years from application with a percentile rank
      of at least 40%
    • Certificate of General Weighted Average
    • Recent 2×2 picture, 2pcs.
    • Long White Folder

          Admission Requirements for Transferees:

    •  Birth Certificate NSO: 1 original, 1 photocopy
    • Authenticated Transcript of Records: 1 original, 1 photocopy
    • Certificate of Good Moral Character: 1 from the Dean
    • Certificate of General Weighted Average
    • NMAT Result percentile rank of at least 40%
    • Honorable Dismissal from previous school
    • Certificate of Eligibility for Admission (CEA)
    • Recent 2×2 recent, 2pcs.
    • Long White Folder

      Admission Requirements for International Students:

    • Three (3) copies of Birth Certificate: 1 original, 2 authenticated
    • Passport with updated student visa, ACR I Card
    • Three (3) Official Transcript of Records: 1 original, 2 copies authenticated
    • Transfer Credentials
    • Certificate of Eligibility for Admission
    • Two (2) Certificates of Good Moral Character (original copies): 1 from the Dean and 1 from a Professor
    • Three (3) copies of Diploma: 1 original, and 2 authenticated
    • Two (2) copies of NBI Clearance
    • Certificate of General Weighted Average
    • Five (5) copies of your recent photo (black and white) 2×2 in size
    • One Hundred Dollars ($ 100.00) application fee. Use international money order or bank draft. Do not send cash.
    • A brief narrative of how your medical education will be financed.
    • Original copy of NMAT result not later than 2 years from application with a percentile rank of at least 40%
    • Long white folder
    • Three thousand pesos (P3,000.00) for College Fund Annual Fee or (P1,500.00) per semester

     Step 2: Psychological – IQ Test

    • Pay Psychological – IQ Test Fee of P1,500 to the any of our designated banks found in the third page through over–the–counter transaction under Account Name:  MATIAS H. AZNAR MEMORIAL COLLEGE OF MEDICINE.
    • Get from the Dean’s Secretary your schedule of Psychological – IQ Test.
    • Proceed to Testing Center located at 2nd floor for Psychological – IQ Test as scheduled.
    • Result of your psychological test shall be forwarded by the Guidance Office to the Dean’s Office.

    Step 3: Interview and Screening

    • Get from the Dean’s secretary your schedule of in-person panel interview.
    • Submit to panel interview and screening (c/o Admission Committee).

    Step 4: Notice of Acceptance

    • Wait for a notice of acceptance that shall be emailed to you by the Dean’s Office and submit for physical examination, laboratory tests and Chest X-ray once you receive it.

    Step 5: Physical Examination, Laboratory Tests and Chest X-ray

    • Submit for physical examination at MHAM Clinic; laboratory tests (CBC, urinalysis, HBsAg, Anti-HBs and drug test) and Chest X-ray PA view at MHAM Diagnostics.
    • Pay laboratory tests and Chest X-ray fee of P 1,200.00 to MHAM Diagnostics.
    • Submit results of the laboratory tests and Chest X-ray to MHAM Clinic.

    Step 6: Enrolment

    • Secure and fill-up the enrollment form from the Dean’s Office after you have submitted for physical examination, laboratory tests and Chest X-ray.

     Step 7: Fees and Validation of Enrolment

    • Enrolment Reservation Fee: P 10,000.00
    • Enrolment Downpayment Fee: P 70,000.00
    • You can pay the enrolment reservation and downpayment fee at any of our designated banks found below through over–the–counter transaction under Account Name:  MATIAS H. AZNAR MEMORIAL COLLEGE OF MEDICINE.

    • Proceed to the Student Account Section (SAS) at the Accounting Office and show your original receipts of payments for validation of your enrolment. Also send a scanned copy of your original receipts as proof of payment to [email protected].


    Step 8: Submission of Enrolment Form.

    • Submit your filled-up enrollment form at the Dean’s Office.
    • You are considered officially enrolled at MHAM College of Medicine once you have submitted the filled-up enrolment form.

    Step 9: College Annual Fund Fee

    • Pay the College Annual Fund Fee at the Student Council’s Office.

    Step 10: Study Load

    • Secure your study load at the Registrar’s Office.
      Make a photocopy of your study load and present it to the Professor when attending classes in the first week.

Old Students

Step 1: Enrolment

  • Visit www.mham.edu.ph and select Online Enrolment and download the enrolment form.
  • Proceed to the Registrar’s Office at the ground floor, ask for your study load and fill up the enrolment form.
  • For returnees, kindly proceed first to the Registrar’s Office at the ground floor for re-evaluation.

Step 2: Payment of Tuition and Other Fees

  • Pay the enrolment fee to any of our designated banks found below through over–the–counter transaction under Account Name:  MATIAS H. AZNAR MEMORIAL COLLEGE OF MEDICINE.

Step 3: Random Drug Test and Psychological Test

  • Submit for random drug testing at MHAM Clinic and psychological test.
  • Pay the drug test fee to MAHM Diagnostics.
  • Pay Psychological Test Fee of P1,500 to the any of our designated banks found above through over–the–counter transaction under Account Name:  MATIAS H. AZNAR MEMORIAL COLLEGE OF MEDICINE.
  • Submit result of the random drug test to MHAM Clinic.
